• Initial Assessment 

In order to understand the depth and scope of your family member's needs, it is necessary to perform a comprehensive in-home assessment.  During this evaluation the client will be assessed using a formal qualitative assessment tool that examines all aspects of their needs which includes, but is not limited to, health related issues, living conditions, level of activity and a benefits checkup.  This initial evaluation takes approximately 3 hours and and the assessment fee is $300.00.

• Hourly Consultation

Also available to assist your family members are hourly visits and other services billed hourly.  

• Referral Services

We maintain a comprehensive list of service providers and agencies that can assist you and your family member.  We can review this list and assist you in the selection of the appropriate services link during any visit or just call us at the number below.
